Approved Provider Program Portal!

The AOTA Approved Provider Program portal houses two programs:

• Application for Recognition as an AOTA Approved Provider: This program is for prospective providers. The program houses the application for prospective providers who wish to apply to become an AOTA Approved Provider.
• Current Provider Applications & Reports: This program is for current AOTA Approved Providers who wish to submit an application, report or renewal.
