First-time users, please register at the top-right of this page. If registering for an organization use a general email address that is transferable to others in the organization (e.g., continuinged@organization.org) Notate your username and password so that you do not forget it.
If you have registered, click the "Log in" button and select "SurveyMonkey Apply" and go to My Applications. If you do not remember your PW use the PW reset feature. AOTA staff are not able to reset your PW.
Application Tips!
- Each section of the application corresponds with the APP Criteria and Guidelines. Read the Guidelines and ensure answers and processes meet the Guidelines.
- Read the Tips for Writing a Process document. Ensure process-related questions are answered with a detailed overarching process. Read the Tips for Writing a Process document.
- The applicant may save work and exit to finish it later but must answer every question on a page before saving or advancing. You may use placeholders if needed.
- The applicant may view the application in its entirety. Download the application by clicking on the 3 dots (top right) and choosing "Download".

Application Review Process:
- Applications are reviewed in the order received and generally within 45 business days of receiving payment (applicants will receive an invoice in 3-5 business days).
- The applicant may request the application be expedited. Review expedited fees.
- The application fee is a processing fee and not an approval fee. AOTA does not grant final approval until payment is made.
- Applicants will receive an automated email when the review is complete. To ensure automated notifications are received please ensure SurveyMonkey Apply email addresses are added to safe sender lists.
Applicants will be denied recognition as an AOTA Approved Provider if there is not sufficient evidence in the application to demonstrate compliance with the APP Criteria and Guidelines. The reasons for denial of recognition will be specified in writing to the applicant. Applicants may ONLY submit clarifications for the Application for Recognition as an AOTA Approved Provider on ONE occasion. A 3rd review may be requested for an additional fee.
